The Correlation Between the Preoperative and Postoperative Hypoalbuminemia and the development of Acute Kidney Injury in terms of the KDIGO Criteria in the Hip Fracture Surgery in Elderly Patients

Objective: This study was planned to determine the correlation between the preoperative and postoperative hypoalbuminemia and the Acute Kidney Injury(AKI) in the hip fracture surgery in elderly patients.
Methods: One hundred and sixty patients who were at the age of 65 years and over were scanned retrospectively. They were grouped into 4 as the preoperative albumin level of <3.8, preoperative albumin level of >3.8, the postoperative 2nd day albumin level of <2.9, and the postoperative 2nd day albumin level of >2.9. In the beginning and 7-day, the age, gender, white blood cell(WBC), hemoglobin(HB), hematocrit(HCT), glucose, blood urea nitrogen(BUN), serum creatinine(sCr), albumin values, fever, anesthesia method, the presence of blood transfusion, surgical period, hospitalization durations and expenses in the postoperative period were recorded for all the patients.
Results: In the study, 92 women and 68 men were scanned. AKI was observed in 28 (17.5%) of all the patients and 16 (57.1%) of them were determined in stage 1. The AKI development in Group 1 and Group 3 was significantly high (p<0.05). Advanced age, multiple drug usage, postoperative glucose level, and the blood product transfusion during the operation were significantly associated with the AKI (p<0.05). The hospitalization period and cost were high in the patients with AKI (p<0.05).
Conclusion: It was determined that hypoalbuminemia was associated with AKI development, the preoperative or postoperative hypoalbuminemia affected the AKI development at similar rates. Advanced age, multiple drug usage, postoperative glucose level, and the blood product transfusion during the operation were associated with the AKI.
